FMLA Act of 1993

Sedgwick now handling benefits cases.

As of March 1st of 2007, Broadspire (the company who handled benefits cases for Bellsouth) was replaced by a new company named Sedgwick. Along the change of the company used, are the policies for reporting short term disability, long term disability, workmens comp and FMLA.

To report your claims for STD/LTD/workmens comp/FMLA, you can contact Sedgwick at 888-488-9559, and fax in any paperwork to 866-299-4151.

Important notice! on FMLA Changes

On December 23 2007, the way your FMLA can/will be reported will be changing. For more information, please contact our Benefits Coordinator at 706-860-3207.

Also, the Company now provides FMLA forms that are to be faxed by your doctor FROM his/her office. It is very important that you have your forms faxed FROM THE DOCTOR'S OFFICE, and keep a copy of the confirmation of the fax! This will help prevent any reported FMLA from being DENIED!
